COVID 2nd Dose Vaccination

Aloha AMS Families,

Thank you to the families that participated in our COVID Vaccination Clinic. It was a success with many students, family, and community members taking advantage. Below is the process for you to sign up for the second dose that is scheduled for Monday, June 14, 2021, to take place at Aliamanu Middle School Cafeteria. If you did not participate, please disregard this message.

When you go in to register for the second dose, it will ask you which type of Vaccine that you received and the date on which you received it. This information can be found on your vaccination card. Please see additional information below along with an attached flyer that has the registration link and a QR Code to aid in registering for the second dose. (If the link should not work, please copy and past it into your internet browser.)

  1. Second dose date will be 21 days from Monday, May 24, 2021. This will fall on Monday, June 14, 2021.
  2. Please bring your CDC vaccination card to your 2nd dose appointment

How to schedule your 2nd dose

  1. Click on “Schedule Options” in either your 1st dose Confirmation OR 1st dose Reminder email and follow the instructions within the hyperlink to schedule the 2nd dose appointment.

COVID Vaccination Clinic at AMS

Aloha AMS Families

We are excited to partner with Safeway to provide COVID-19 vaccinations for students and family members age 12 and up. If you are participating in this vaccination opportunity you can click here www.mhealthsystem.com/Aliamanu to make an appointment. 

For all 12+ AMS students who have signed up for vaccination during school hours, a signed consent form from parents and guardians is sufficient.

For individuals age 12-15: During non-school hours, they will need the company of a parent, guardian, or non-custodial adult in addition to signing the consent form. 

For 16-18 years old, we will need just the signature of parents or guardian on the consent form.

Please print and fill it out the consent form found below. All students MUST bring completed and signed consent forms on the day of the vaccination, Monday, May 24, 2021. 

Reminders for Participants

Individuals receiving vaccination must:

  • Complete and bring a printed copy of the consent form with parent’s signature
  • Bring Student ID
  • Provide proof of insurance if possible
  • Wear a mask
  • Be in good health on the day of vaccination

Borrowed Device Return Request

Aloha AMS Families

If you have borrowed either a Chromebook and or a hotspot from our school, you can return them to the front office starting Friday, May 21 to the front office. If your child(ren) still needs the devices to complete their work over the weekend, the device return will continue on into the last week of school.

Aloha AMS Families

As this school year is quickly coming to a close, we are working on a few exciting activities to celebrate our students’ hard work and resilience this year. Because of the celebration activities, there will be a schedule change for the last week of school. We truly apologize for any inconvenience.  

On Thursday, May 27th, ALL 8th graders (8A and 8B) will come to school to celebrate together with a fair and other fun activities. On Wednesday, May 27th, all 8th graders will be at home while all the 7th graders are on campus. Due to Hawaii Department of Education restrictions, this year all of our celebration/promotion activities will be limited to students only. 

Monday, Tuesday, and Friday of that week will remain the same. Monday will be an A day, Tuesday a B day, and Friday will remain an online day with a shorter modified schedule.

If you have any questions, please feel free to call the office at 421-4100. Once again, thank you for your continued support through this school year. We are looking forward to celebrating with our students during these last weeks of school.

Honoring Your Child:  Bringing Out the Best in Them

TUESDAY, APRIL 13TH AT 9:00: Join us for a parent presentation called, “Honoring Your Child:  Bringing Out the Best in Them.”

Join FOCUS Hawaii for an interactive parent workshop in celebration of April’s Month of the Military Child! Learn how to bring out the best in your middle schooler by communicating feelings, promoting emotional regulation, and “getting to the green.”
