Aliamanu Middle students and parents!
Our Registration Office is located inside the main Administration building.
Registration hours are from 8:00 a.m. – 11:30 a.m., Monday to Friday.
For enrollments outside the registration hours, appointments are recommended. Please email Mary_Ann.Kurose@k12.hi.us
Please allow our office at least 2 days to process completed registrations.
CURRENT STUDENTS WHO ARE WITHDRAWING:
Is your family moving during the school year? Allow our office at least 2 days to prepare the release packet for your child’s next school. Release packets may take up to 7 business days to prepare depending on the time of the year. Complete a Request for Release and submit it to the front office as soon as possible.
REQUIRED DOCUMENTS for AMS Enrollment
If you are registering a new student, we will collect the following:
- Hawaii DOE Student Enrollment form (SIS-10W)
- Proof of Current Address (e.g., utility bill or housing papers) to certify district residency
- Student’s Birth Certificate
- Parent/Guardian’s Photo ID (driver’s license, passport, state ID)
- Legal Documents (If applicable):
• Power of Attorney (if you’re not the legal parent)
• Divorce Decree
• Temporary Restraining Order
• Legal name change
• Court orders - IEP/504 plans (if applicable)
- Official Release Packet from the previous school
(If you don’t have it, we can request it.) - Health and Immunization Requirements
Health Requirements should be recorded on the Hawaii Department of Education’s School Health Record (Form 14).- Tuberculosis (TB) Clearance form: Required for students new to Hawaii (upon first enrollment in a Hawaii school)
- Confirmation of negative TB Risk Assessment & Symptom Screen, negative TB Skin Test, OR negative Chest X-ray
- Physical Exam (wellness-child exam):
Required for all incoming 7th-graders and “new to Hawaii” 8th graders- Exam to be completed within 12 months prior to the start of the applicable school year
- OR submit a medical appointment slip signed by a health professional
- Up-to-date Immunizations – required for all students
- Tuberculosis (TB) Clearance form: Required for students new to Hawaii (upon first enrollment in a Hawaii school)

MEDICATIONS:
- Please contact the Health Aide at (808) 421-4100 ext. 230 for information on the requirements to administer daily medication. Only approved medication from a physician and Public Health Nurse will be administered. Information for use of Epi-Pens may be found on the HI DOE’s website.
- Students in possession of drugs/medications or using drugs/medications without meeting the above conditions may be subject to disciplinary action. Medication should be taken in the office.
