The Registration Office is located inside the main Administration building. Look for the door between the shark and bamboo.

Office hours are from: 8:00am – 11:30am, Monday to Friday

All incoming students generally start school the day after completing registration.

General Requirements:

If you are registering a new student at Aliamanu Middle, please make sure you have the following ready for us:

  1. Proof of Residence (e.g., utility bill or housing papers) to certify district residency
  2. Birth Certificate
  3. Power of Attorney/Guardianship papers – if the last name is different or you’re not the legal parent
  4. Hawaii DOE’s Form 14 Health Record pdf file Student Health Record Form
    • T.B. Clearance – required for students new to Hawaii (upon first enrollment in a Hawaii school)
      • confirmation of:
        1. negative TB Risk Assessment & Symptom Screen
        2. negative TB Skin Test, OR
        3. negative Chest X-ray
    • Physical Exam (wellness-child exam) – required for all incoming 7th graders and “new to Hawaii” 8th graders
      • exam to be completed within 12 months prior to the start of the applicable school year
      • OR need to submit a medical appointment slip signed by a health professional
    • Up-to-date Immunization Records – required for all students
      • DtaP or DTP (Diphtheria, Tetanus, Pertussis;): 5 doses
      • Polio: 4 doses
      • Hepatitis B: 3 doses
      • Hepatitis A: 2 doses
      • MMR (Measles, Mumps, Rubella): 2 doses
      • Varicella (Chickenpox): 2 dose
      • HPV (Human Papillomavirus Vaccine): 2 doses
      • MCV (Meningococcal Conjugate Vaccine): 1 dose
      • Tdap (Tetanus, Diphtheria, Pertussis): 1 dose

OR Need to submit a medical appointment slip signed by a health professional

  • Health Requirements should be recorded on the Hawaii Department of Education’s School Health Record (Form 14). This form can be obtained from the HI DOE’s website, the school’s front office, or from this link (
  • The school has a health room where students may receive minor attention. In cases of illness or serious injury, parents are notified and asked to pick up their child.
  • Please contact the Health Aide at (808) 421-4100 ext. 230 for information on the requirements to administer daily medication. Only approved medication from a physician and Public Health Nurse will be administered. Information for use of Epi-Pens may be found on the HI DOE’s website.
  • Students in possession of drugs/medications or using drugs/medications without meeting the above conditions may be subject to disciplinary action. Medication should be taken in the office.
